LOKOMAT (YÜRÜME ROBOTU)

Robot teknolojisininkullanıldığı alanlardan biri de Rehabilitasyondur. Kollarını kullanamayan hastalar için kol robotları, sanal gerçeklik teknolojisi ile birleştirilerek hastaların el ve kol becerilerini artırmayı amaçlamaktadır.

Yürüyemeyen hastalar için kullanılan en yaygın robot örneği Lokomat adıyla tanınan yürüme robotlarıdır. Bu sistemde hastaya, bir yürüme bandının üzerinde askılar vasıtasıyla yukarı alındıktan sonra , bacakların iki yanına yerleştirilen robotik kısımlar yardımıyla normal yürüyüş şekline birebir benzer tarzda yürüme hareketi yaptırılmaktadır. Vücut ağırlığı, yürümenin temposu ve şekli bilgisayar programıyla kontrol edilebilmekte, hasta karşısındaki aynadan veya sanal gerçeklik ekranından durumunu izleyebilmektedir. Sistemin en önemli avantajı, normal yürümeye çok benzeyen şekilde hareketler yaptırması ve beyindeki merkezleri sürekli uyarmasıdır. Bu yöntemle hastaların iyileşme süreci hızlandığı gibi yürüme şekilleri de normale yakın biçimlerde gelişmektedir.

Robotlar, hastanın egzersize uyumu ve performansıyla ilgili verileri toplama özelliğine sahip olduğundan , terapist hastanın yanında olmasa dahi , yaptığı egzersizler konusunda bilgi edinebilmekte, önceki durumuyla sonraki durumunu rahatça kıyaslayabilmektedir.

Lokomat yürüme robotunun en çok kullanıldığı rahatsızlıkların başında inme'ye bağlı yarım felçler, omurilik felçleri, mültipl sklerozve ALS gibi dejeneratif hastalıklar gelir. Son yıllarda çocuklar için özel olarak yapılmış "Pediatrik Lokomatlar" bu tedaviden küçük çocukların da yararlanmasına imkan sağlamıştır. HASTA VE ÇALIŞAN GÜVENLİĞİNİN SAĞLANMASINA DAİR YÖNETMELİK

BİRİNCİ BÖLÜM
Amaç, Kapsam, Dayanak ve Tanımlar

Amaç
MADDE 1 - (1) Bu Yönetmeliğin amacı; 2 nci madde kapsamındaki tüm sağlık kurumlarında, hasta ve çalışan güvenliği için güvenli hizmet sunumu ve güvenli bir ortam sağlanmasına, hizmet sunumunda kalitenin artırılmasına, sağlık kurumunda hasta ve çalışanlar için muhtemel risklerin belirlenmesine, bu risklerin giderilmesi için uygun yöntem ve tekniklerin belirlenmesine ve hizmet içi eğitimler ile güvenli hizmet sunumu ve güvenli çalışma ortamının sürdürülebilirliğinin sağlanmasına yönelik usul ve esasları düzenlemektir.

Kapsam
MADDE 2 - (1) Bu Yönetmelik kamu, üniversite ve özel sektör ikinci ve üçüncü basamak sağlık kurumlarını kapsar.
Dayanak
MADDE 3 - (1) Bu Yönetmelik, 7/5/1987 tarihli ve 3359 sayılı Sağlık Hizmetleri Temel Kanununun 3 üncü maddesinin birinci fıkrasının (e) ve (f) bentlerine, 13/12/1983 tarihli ve 181 sayılı Sağlık Bakanlığının Teşkilat ve Görevleri Hakkında Kanun Hükmünde Kararnamenin 43 üncü maddesine dayanılarak hazırlanmıştır.

Tanımlar
MADDE 4 - (1) Bu Yönetmelikte geçen;
a) Bakanlık: Sağlık Bakanlığını,
b) Çalışan: Hizmetin, resmî veya özel sağlık kurumlarında, sağlık hizmetinin verilmesine iştirak eden bütün sağlık meslekleri mensuplarını ve sağlık meslekleri mensubu olmasa bile sağlık hizmetinin verilmesine sorumlu olarak iştirak eden ve hizmetin verilmesine destek sağlayan kimseleri,
c) Daire Başkanlığı: Tedavi Hizmetleri Genel Müdürlüğü Performans Yönetimi ve Kalite Geliştirme Daire Başkanlığını,
ç) Değerlendirici: Daire Başkanlığı tarafından görevlendirilen Hizmet Kalite Standartlarının sağlık kurumlarındaki etkinliğini değerlendiren kişiyi,
d) Değerlendirme: Daire Başkanlığı tarafından sağlık kurumlarında Hizmet Kalite Standartlarının etkinliğinin ölçülmesi faaliyetini,
e) Hasta: Sağlık hizmetlerinden faydalanma ihtiyacı bulunan kimseyi,
f) Hasta ve çalışan güvenliği: Sağlık hizmeti sunumunda hasta ve çalışanların zarar görmesine yol açabilecek her türlü işlem ve süreçler ile ilgili alınacak tedbir ve iyileştirme uygulamalarına yönelik faaliyetleri,
g) Hizmet Kalite Standartları (HKS): Daire Başkanlığı tarafından sağlık kurumlarının hizmet sunumuna yönelik hazırlanan standartları,
ğ) Sağlık kurumu: Kamu, üniversite ve özel sektör ikinci ve üçüncü basamak sağlık kurumlarını,
ifade eder.

İKİNCİ BÖLÜM

Temel Esaslar, Hasta ve Çalışan Güvenliği Uygulamaları
Temel esaslar
MADDE 5 - (1) Bu Yönetmelik kapsamındaki sağlık kurumları; bu Yönetmelik hükümleri doğrultusunda uygulama yapmak, hasta ve çalışan güvenliğini sağlamaya yönelik gerekli faaliyetleri gerçekleştirmek amacıyla, kendi iç düzenlemelerini yapmak ve tedbirleri almakla yükümlüdür. Sağlık kurumları, hasta ve çalışan güvenliğine ilişkin tedbirleri alırken ve düzenlemeleri yaparken Daire Başkanlığınca hazırlanan ve Bakanlığın resmî internet sayfasında yayımlanan Hizmet Kalite Standartlarını esas alır.
Hasta güvenliği uygulamaları
MADDE 6 - (1) Bu Yönetmelik kapsamındaki sağlık kurumları;
a) Hasta kimlik bilgilerinin tanımlanması ve doğrulanması,
b) Hastaya uygulanacak girişimsel işlemler için hastanın rızasının alınması,
c) Sağlık hizmeti sunumunda iletişim güvenliğinin sağlanması,
ç) İlaç güvenliğinin sağlanması,
d) Kan ve kan ürünlerinin transfüzyon güvenliğinin sağlanması,
e) Cerrahi güvenliğin sağlanması,
f) Hasta düşmelerinin önlenmesi,
g) Radyasyon güvenliğinin sağlanması,

ğ) Engelli hastalara yönelik düzenlemelerin yapılması,
hususlarında gerekli tedbirleri alır ve düzenlemeleri yapar.

Çalışan güvenliği uygulamaları
MADDE 7 - (1) Bu Yönetmelik kapsamındaki sağlık kurumları;
a) Çalışan güvenliği programının hazırlanması,
b) Çalışanlara yönelik sağlık taramalarının yapılması,
c) Engelli çalışanlara yönelik düzenlemelerin yapılması,
ç) Çalışanların kişisel koruyucu önlemleri almasının sağlanması,
d) Çalışanlara yönelik fiziksel saldırıların önlenmesine yönelik düzenleme yapılması,
hususlarında gerekli tedbirleri alır ve düzenlemeleri yapar.

Hasta ve çalışan güvenliği ortak uygulamaları
MADDE 8 - (1) Bu Yönetmelik kapsamındaki sağlık kurumları;
a) Enfeksiyonların kontrolü ve önlenmesi ile ilgili olarak;
1) Enfeksiyonların kontrolü ve önlenmesine yönelik bir programın hazırlanması,
2) Hastane enfeksiyonlarının izlenmesine yönelik sürveyans çalışmalarının yapılması,
3) El hijyeninin sağlanmasına yönelik düzenlemelerin yapılması,
4) İzolasyon önlemlerinin alınması,
5) Temizlik, dezenfeksiyon ve sterilizasyon uygulamalarının gerçekleştirilmesi,
b) Laboratuvar güvenliği ile ilgili olarak;
1) Biyogüvenlik düzeyine göre gerekli önlemlerin alınması,
2) Laboratuvarda çalışılan testlerin kalite kontrol çalışmalarının yapılması,
3) Panik değerlerin bildiriminin sağlanması,
c) Radyasyon güvenliği ile ilgili olarak; tanı ve tedavi amaçlı radyoaktif madde kullanılan alanlarda koruyucu önlemlerin alınması,
ç) Renkli kod uygulamaları ile ilgili olarak;
1) Mavi kod uygulamasının yapılması,
2) Pembe kod uygulamasının yapılması,
3) Beyaz kod uygulamasının yapılması,
d) Güvenlik raporlama sistemi ile ilgili olarak;
1) Güvenlik raporlama sisteminin kurulması,
2) Bildirimi yapılacak olayların asgari ilaç güvenliği, transfüzyon güvenliği, cerrahi güvenlik, hastaların düşmesi, kesici delici alet yaralanmaları, kan ve vücut sıvıları ile temas konularını kapsaması,
3) Bildirimi yapılan olaylar ile ilgili düzeltici ve önleyici faaliyetlerin uygulamaya konulması,
e) Eğitimler ile ilgili olarak; sağlık kurumlarında görevli tüm çalışanlara, hasta ve çalışan güvenliği konusunda eğitimlerin verilmesi,
f) Komitelerin kurulması ile ilgili olarak;
1) Hasta güvenliği komitesi,
2) Çalışan güvenliği komitesi kurulması,
hususlarında gerekli tedbirleri alır ve düzenlemeleri yapar.

ÜÇÜNCÜ BÖLÜM

Çeşitli ve Son Hükümler
Sorumluluk
MADDE 9 - (1) Bu Yönetmelikte yer alan usul ve esasların kesintisiz ve etkin bir şekilde uygulanması, uygulama sırasında tespit edilen sorun ve aksaklıkların giderilmesi için gerekli tedbirlerin alınması, uygulamanın takip edilmesi ve değerlendirilmesi, ilgili sağlık kurumunun en üst amirinin sorumluluğundadır.
Değerlendirme
MADDE 10 - (1) Bakanlık bu Yönetmelik kapsamındaki sağlık kurumlarını hasta ve çalışan güvenliği açısından değerlendirir. Sağlık kurumlarının değerlendirilmesi sürecinde Hizmet Kalite Standartları kullanılır. Değerlendirmeler değerlendiriciler tarafından yılda en az bir defa Hizmet Kalite Standartlarından seçilecek bölümler üzerinden yapılır. Sağlık kurumlarının bu kapsamda değerlendirilen bölümlerde yer alan hasta ve çalışan güvenliği ile ilgili standartlardan 100 üzerinden en az 80 puan alması gerekmektedir. Bakanlık bu puanı artırmaya yetkilidir.
(2) Hasta ve Çalışan Güvenliği Değerlendirme Puanı aşağıdaki şekilde hesaplanır:
